Due to a large number of late cancellations, we now require that a credit card be put on file for your first visit. If you cancel with fewer than two business days’ notice, your credit card will be charged the full cash price of the appointment.
Please download and fill out the New Patient Intake Form and bring it to your initial visit.
If you are insured, even if you don’t believe services will be covered, please also download, fill out, and bring the Insurance Benefits Form.
PLEASE NOTE: Not all Cigna and Harvard Pilgrim plans cover Dr. Galbraith. You are responsible for understanding your coverage, and for paying any costs incurred that are not covered by insurance. Please call the office if you need help understanding your benefits, but please call your insurance company first, as they are the experts on your plan.
You may see Dr. Galbraith for any acute or chronic complaint for which you would see a general family practice doctor, but she will not be considered a Primary Care Physician (PCP) by your insurance.

What to Expect
Your first appointment with the doctor will be about 90 minutes long. She will take a thorough history of your current complaints, and will want to know about your medical history and that of your family. She may order appropriate lab tests, and will want to see the results of any testing you’ve had done in the past 2 years, especially if relevant to your current complaint. She will perform a thorough physical exam, and will formulate a health plan based on your individual needs.
To your first appointment, you should bring:
- Completed New Patient Intake Form.
- Completed Insurance Benefits Form.
- A bag containing all medications and supplements you currently take, whether they be pills, liquids, creams, sprays, patches or otherwise delivered into your body.
- Copies of labs done in the past 2 years, if you can get them in time.
Follow-up appointments are 30 minutes in length and involve discussion of your progress on the current health plan, results of a set of lab work that was ordered, a new health complaint, or adjustments to the health plan. Longer appointments may be available for cash patients at the Doctor’s discretion.
Note: Because many of our patients are very sensitive to perfumes and chemical odors, please avoid wearing perfumes or strong smelling lotions to your appointment, in order to honor those with chemical sensitivities.
